How Does The System At The Hospital In Scrubs Work?

I understand that interns are the newest and than there are the residents and than attendees.
But how does it work with every doctor getting interns, and than some getting residents?
SOMEONE EXPLAIN PLEASE!

    It is only residents that actually look out for interns and mentor them. Some fully qualified doctors just get them to help out so they can learn for themselves and have first hand experience.
