It looks very good (be sure to bold headings, of course)! You have all the necessary information and from an employer’s point of view, your experience looks good.

I would use some caution posting personal information like this online in such a public forum because of the unfortunate fact that some people collect information like this for less than helpful purposes!

You might have already considered that.
